MDS Coordinator (Part-Time / PRN) San Francisco Post Acute Pay Range: $50.00 - $55.00 per hour (Part-Time / PRN) San Francisco Post Acute is seeking an experienced MDS Coordinator to oversee the resident assessment process and ensure compliance with federal, state, and facility guidelines. This role is responsible for coordinating MDS assessments, supporting PDPM optimization, monitoring quality outcomes, and collaborating with the interdisciplinary team to deliver exceptional resident care. Highlights Flexible Part-Time / PRN schedule Competitive pay: $50-$55 per hour Opportunity to impact quality measures and reimbursement outcomes Collaborative interdisciplinary healthcare environment Key Responsibilities Oversee and coordinate the completion of MDS assessments, including Medicare, quarterly, annual, and significant change assessments, ensuring accuracy and regulatory compliance. Monitor PDPM performance, quality measures, and clinical outcomes, providing recommendations to improve resident care and facility performance. Collaborate with nursing and interdisciplinary teams to develop, review, and update individualized care plans based on resident assessments. Provide MDS and PDPM education, support, and guidance to staff while maintaining accurate documentation, data submission, and resident records. Qualifications Active RN license in good standing in the State of California. Graduate of an accredited nursing program; Associate Degree in Nursing or higher preferred. Experience with MDS, PDPM, Medicare reimbursement, and long-term care regulations preferred. Long-Term Care and skilled nursing experience strongly preferred.
