• Provide one-on-one care and support to residents, including assistance with daily living activities

• Monitor and report changes in residents' condition, including physical, emotional, and cognitive status
• Collaborate with other care team members to develop and implement individualized care plans
• Maintain accurate and detailed records of resident care and activities
